Dr. Ravinder Sohi relocated to Port Credit in South Mississauga since June 1, 2024.

For Patients of Dr. Mang or Dr. Chan:

1. May 31, 2024 was the last day for medical appointments. The office is now closed.
2. Patients who ordered copies of their medical records prior to closing the office have had their charts reviewed and copied onto a USB. Both Dr. Chan and Dr. Mang have completed this process, and all those patients were contacted directly with instructions to pick up their USB at the pharmacy in our building (Professional Medical Pharmacy). Please bring your Health Card for identification when picking up your USB.  Please note USB’s will not be distributed by any other means, including to other physicians or clinics. USB’s must be picked up in person directly by patients with photo ID or individuals designated to do so with written, signed authorization by patients.
   * Dr. Mang’s patients who have not yet picked up their USB’s from the Professional Medical Pharmacy must do so by January 31, 2025. After this date, all remaining USB’s will be removed from the pharmacy. Please pick up your records as soon as possible. After January 31, 2025, you may contact RSRS directly to order your records (see below).  
   * Dr. Chan’s patients who did not previously order or pick up their USB’s may also obtain their records from RSRS (see below).
3. For patients who did not order a USB of medical records or who do not pick up USB’s :
   • *For both Dr. Mang’s and Dr. Chan’s Patients:  Effective immediately, all new requests for a copy of your medical chart should be made through RSRS.  Please note your chart will not include any reports received after July 11/24. The contact information for RSRS is as follows:
